This is an adaptation of Lament01's tab.  Neither his nor mine is entirely accurate, 
but I made a few changes and clarifications that I thought might be helpful to others.  
This is how I play it.  Again: THIS IS NOT AN EXACT TRANSCRIPTION, BUT AN ACOUSTIC ADAPTATION

(Capo on 6th fret) 
Note:  don't play the top 'e' string throughout; it just sounds bad

Chords used:

Cadd6 (032210)
D     (200232)
Em    (022000)
G     (320033)

Also, throughout the song there is a little flourish that comes at the end of some of the 
lines.  I've tabbed it as "G C G" but it's easiest to play by just moving the fingers you've 
got on the low E and A strings up and then back down, like this:
