I just took a close look at the version I posted earlier, and found some 
errors and ommission.  This is closer to the "real" thing.  The ninth, 
eleventh, thirteenth, and other "masochistic" chords have been simplified 
to sevenths.

- Period (.)   =  1/8 note rest at start of a measure
- Underline(_) =  sustain note into next measure
